If you would like to receive your Form 1098-T electronically, please give your consent by January 1, 2020 using the link provided below, and follow the step by step instructions. Form 1098-T will be available online before the end of January 2020. If electronic consent is not received by January 1, 2020, Form 1098-T will be mailed to the current address on file.
By submitting the information below, you are giving your consent to receive your 1098-T statement electronically. By signing up for an electronic version, no paper 1098-T statement will be mailed to you. When your statement is ready, an email will be sent to you with instructions on how to access your 1098-T statement online.
